Refactored request, auth, endpoint:

- zap.Request : refactored into its own file, along with supporting
  types and functions (e.g. http params related)
    - added setContentTypeFromFilename thx @hauleth.
- zap.Auth : zap.Auth.Basic, zap.Auth.BearerSingle, ...
- zap.Endpoint : zap.Endpoint, zap.Endpoint.Authenticating
